Method autoplastic with burn injuries in children

Defining indications for autoplasty, caring about the maximum engraftment (and, therefore, saving the patient’s life), we attach great importance to the preoperative preparation, which aims at the normalization of KHS and water-electrolyte metabolism (the method of infusion-transfusion therapy) to increase protective properties of the organism, which is achieved by the introduction of agents for passive immunity (gamma-globulin and hyperimmune plasma), and in recent years is the use of reinfusion of UBI and hemosorption.

Important to consider the impact on microbial flora (antibiotics are trying to assign antibiotikogramma) and cleansing the wound from necrotic and purulent masses, although, however, the presence of pus in a wound is not a contraindication to skin grafting.

We believe it is necessary to emphasize the high positive effect of the implementation in daily practice of the method of open management of children with burns. We use plastic frames with ventilation open wound by a stream of dry warm air that blocks the development of pyogenic flora, promotes healing of wounds under a dry scab.

As for necrosectomy, we preferred its chemical variant (necrogenesis ointment). We are supporters of the well-known point of view that the treatment of burn disease should be comprehensive, where the operational closure of the burn wound is perhaps the most effective among all available modern methods of treatment of this extremely serious disease.

Getting the surgical treatment of burns, we have often experienced considerable difficulties with the abstraction of the skin. To a certain extent the problem can be solved fine way of autoplastic.

Many years working with this method, we are able to see a big authorizing the possibilities of plastics brands, the main one is a momentary closure of large areas of the lesion (biopsy transplant carried out by a glutinous dermatome model 205 or electric dermatome — MRTU — 422528-65).

Alarmed, however, by the fact that between brands are often formed rough keloid scars, leading to dysfunction of the limbs, various deformations, decreased cosmetic effect.
